1.Lockout tagout basically consists in any individual exercising “total control” over the locking of machines, equipment, processes, or circuits that he repairs and maintains.

 

Authorized personnel/affected personnel
6. Authorized personnel shall understand and be able to implement all aspects of locking/listing procedures. Affected persons shall understand and respect Lockout tagout and shall not attempt or move Lockout tagout used by others.

 

Effective training
3. Understanding of locking responsibilities, procedures, methods, and requirements is derived from effective Lockout tagout training. Knowledge gained through training is reflected in field/operational practices.

 

Validation of LOTO
5. Ensure that machines, equipment, procedures or circuits are in a “zero energy state” before commencing operation or maintenance. The best aversion must be taken to determine that all energy has ceased, been released, dissipated, or discharged.

6. A special kit is required, including key, lock, multi-lock tagout, red tag and shift tag.

7. Complete Lockout tagout is always the first choice. The establishment of alternative methods must be based on the risk assessment of machines, equipment, processes and circuits.

 

Risk assessment
8. Risk assessment is used to discover the safest possible conditions for individual operations. The risk assessment must include the identification and enforcement of control measures so that other regulatory requirements can be met.

 

Shift or personnel change
9. The maximum allowable time for each Lockout tagout is whichever is shorter: one shift or task end. It is important to ensure the integrity of the Lockout tagout discipline by using direct Lockout tagout handoffs, conversion locks, or other appropriate means.

 

The LOTO of contractual action
10. The Company first up and then down: appoint a representative authorized by the Company to perform the Lockout tagout step, at which time external service personnel or contractors shall attach their own personnel to the same energy separation device that the Company representative has locked and secure it.
